Life & Wellness Coaching Agreement & Consent Form

1. Nature & Scope of Coaching Life and Wellness Coaching is a collaborative, goal-oriented process designed to assist individuals and couples with personal growth, lifestyle alignment, stress management, and relationship dynamics.

Non-Clinical Services: Coaching is not licensed psychotherapy, medical diagnosis, mental health treatment, or medical advice. It does not replace care provided by licensed physicians or mental health professionals.
Forward-Focused Approach: Sessions emphasize actionable strategies, forward movement, personal accountability, and positive growth rather than psychological clinical treatment.

2. Client Responsibility & Autonomy The client retains full decision-making authority over their life, relationships, health, and personal choices. The outcomes of coaching depend on the client’s active participation and implementation of strategies discussed during sessions.

3. Dual-Client & Couples Coaching Policy When participating in couples or joint coaching, information shared in individual communications may be brought into joint sessions if relevant to mutual goals. Both partners agree to foster open, honest, and respectful communication during joint work.

4. Separation from Naturopathic Services If you are also receiving Naturopathic or HTMA consultations through our practice, you acknowledge that Life & Wellness Coaching operates under a separate scope of service focused purely on lifestyle and personal coaching goals.

5. Confidentiality & Limits Information shared within coaching sessions is strictly confidential and will not be disclosed to third parties without your written consent, except where required by law (e.g., immediate risk of harm to self or others, or mandated reporting).

Confidentiality & Financial Terms: All session communications are strictly confidential except as required by law. Cancellations require at least 24 hours’ notice.
